Before you start

Gather these so you don't have to stop partway through:

  • Your full legal name (as it appears on government documents)
  • Your date of birth
  • The last 4 digits of your Social Security number (or a Tribal ID if you live on Tribal lands)
  • Your home address (must be a residential address; a P.O. box alone doesn't qualify)
  • A way to prove eligibility — either a qualifying program (SNAP, Medicaid, etc.) or proof of household income
  • A computer or phone with internet, and about 15–20 minutes

If you don't have a Social Security number, you can use a government-issued Tribal ID instead.

Step-by-step

1. Go to the National Verifier

Open checklifeline.org/lifeline.

2. Click Apply Now

3. Confirm your state of residence

4. Enter your name and date of birth

Use your legal name exactly as it appears on government documents.

5. Enter the last 4 digits of your SSN (or your Tribal ID)

6. Enter your home address

This must be where you actually live. If you're without a permanent address, you can use a shelter address or a temporary address.

7. Enter a mailing address if different

If USAC needs to mail you anything, this is where it goes.

8. Confirm your household status

A "household" is everyone who lives together and shares income and expenses. Only one Lifeline benefit per household.

9. Choose how you qualify

Pick one:

  • Through a qualifying program: SNAP, Medicaid, Federal Public Housing Assistance, SSI, Veterans Pension or Survivors Pension, or a Tribal-specific program.
  • Through income: household income at or below 135% of the Federal Poverty Guidelines.

10. Upload documentation if asked

USAC tries to verify you automatically. If it can't, it asks you to upload proof:

  • Program-based: a benefits letter, benefits screenshot, or notice from the agency. It must show your name, the program, the agency, and a date in the last 12 months.
  • Income-based: recent pay stubs, tax return, or other income documents.

11. Sign the certification

You're agreeing your answers are accurate and you understand only one benefit per household.

12. See your result

USAC tells you one of three things:

  • Qualified — Save your application ID and contact Thrive Mobile to apply your benefit.
  • Pending Documentation — Upload the requested documents and check back.
  • Not Qualified — See If you're found not eligible for next steps.

After you're approved

Contact Thrive Mobile with your application ID. We'll apply your benefit to your account, and your Essential plan becomes $0/month.

If you currently have Lifeline with another carrier, applying it to Thrive moves it from them to us. Your old carrier may text or call to confirm.

How long does the application take?

Most online applications get a result in a few minutes. If you need to upload documents, it can take a few more days.

Does USAC keep my Social Security number?

USAC only collects the last 4 digits and uses them to confirm your identity.

What if my address is on Tribal lands?

You may qualify for an enhanced Lifeline benefit and a Tribal-specific plan. The application asks about this automatically.
