How long it takes

The paper application arrives in the mail in 7–10 business days. Plan for a few weeks total once you fill it out and mail it back to USAC.

How to request a paper application

Option 1 — We'll request it for you

Contact Thrive Mobile support. We'll send the request to USAC on your behalf. We'll need:

  • Your full legal name
  • Your mailing address
  • Your preferred language (English or Spanish)

Option 2 — Request it yourself

Email USAC directly at Lifelinesupport@usac.org with:

Subject: Paper Lifeline Application Request

Hello USAC Lifeline Support,

I would like to request a paper Lifeline application be mailed to me.

  • Full Name: [your full legal name]
  • Mailing Address: [street, city, state, ZIP]
  • Preferred Language: [English or Spanish]

What to expect

  1. USAC mails you the application in 7–10 business days.
  2. You fill it out and mail it back with proof of eligibility (a benefits letter, a benefits screenshot from a government portal, or income documents from the last 12 months).
  3. USAC reviews and replies by mail. Decisions usually take a few weeks.
  4. If you're approved, contact us and we'll apply your benefit to Thrive Mobile.

  • It's been more than 2 weeks and the application hasn't arrived
